Gordon D. Jensen, M.D., co-creator of the film, was affiliated with the Department of Psychiatry and the Regional Primate Research Center at the University of Washington in Seattle.

The film is a case study of John, a four year-old child who displays symptoms of what is termed an "infantile psychosis." Film footage of the child spans the first four years of his life, and narration is used to explain the ways in which John's behavior differs from others in his age group. Strategies used to address this "psychosis" are discussed and displayed, and a prognosis is given at the close of the film.

The film begins with preschool children interacting with one another on a playground, accompanied by narration that details the typical behavior of a three year-old. A child named John -- also three -- is then shown playing alone on the same playground, and he is described as having an "infantile psychosis." The symptoms of this "psychosis" are detailed by the narration as we see John continue to play in ways that differ from the other children in his class. Home movie footage of John at 12-36 months is shown, and further narration explains the early signs of the diagnosis. The film concludes with observations of John at age four, and the narration explains that, despite some progress in the development of behavioral skills, the prognosis for the child remains "poor."
